Privacy Policy
Effective Date: August 3, 2026
Columbia Cove (“Columbia Cove,” “we,” “us,” or “our”) operates the website located at https://columbiacovewa.com (the “Site”). Columbia Cove is a licensed supported living provider serving adults with intellectual and developmental disabilities in Clark County, Washington. This Privacy Policy explains what information we collect through the Site, how we use and share it, and the choices and rights you have.
By using the Site, you agree to the collection and use of information as described in this policy.
Information We Collect
Contact Form
When you submit our Contact Us form, we collect your full name, email address, phone number (if provided), and the message you send us, along with your consent to be contacted about your inquiry.
Referral Form
Our “Make a Referral” button links to a secure form hosted by our operations partner, ClickUp. Referral forms may include information about you and, if you are referring someone else, information about the person being referred — which can include contact details and information relevant to their care needs, such as health, behavioral, or disability-related information. We collect this information only to evaluate and respond to the referral.
Employment Applications
Our Careers page links to job application forms hosted by ClickUp. These forms collect the information you choose to provide as part of applying for a position with us, such as your name, contact information, work history, and any documents you upload (e.g., a resume).
Information Collected Automatically
Like most websites, our Site may automatically log standard technical information when you visit, such as your IP address, browser type, device type, pages viewed, and the date and time of your visit. This information is collected through standard web server logs and/or cookies used to operate the Site.
Cookies
We use cookies and similar technologies to help our Site function properly (for example, remembering your preferences and understanding how visitors use the Site). You can control or disable cookies through your browser settings; doing so may affect how some parts of the Site work.
How We Use Your Information
We use the information we collect to:
- Respond to inquiries submitted through our Contact Us form
- Evaluate and respond to referrals submitted through our referral process
- Review and process employment applications
- Operate, maintain, and improve the Site
- Communicate with you about your inquiry, referral, or application
- Comply with legal, licensing, and regulatory obligations as a Washington State supported living provider
We do not sell your personal information, and we do not use information submitted through referral or contact forms for advertising or marketing to third parties.
How We Share Information
We do not sell or rent your personal information. We may share information with:
- Service providers who help us operate the Site and process forms, including ClickUp (which hosts our referral and employment application forms) and our email and web hosting providers. These providers are only permitted to use your information to provide services to us.
- Regulatory and licensing bodies, such as the Washington State Department of Social and Health Services (DSHS) or Developmental Disabilities Administration (DDA), where required for licensing, referral, or compliance purposes.
- Legal or safety reasons, if we believe disclosure is necessary to comply with the law, protect someone’s safety, or respond to a valid legal request.
- A successor entity, if Columbia Cove is involved in a merger, acquisition, or sale of assets, subject to the terms of this policy.
Data Retention
We retain information collected through our forms only as long as necessary to respond to your inquiry, referral, or application, or as required by applicable licensing, regulatory, or legal recordkeeping requirements, after which it is securely deleted or de-identified.
Security
We use reasonable administrative, technical, and physical safeguards designed to protect the information you provide to us. However, no method of transmission over the internet or electronic storage is completely secure, and we cannot guarantee absolute security.
Your Privacy Rights
Depending on where you live, you may have rights regarding your personal information, which can include the right to:
- Request confirmation of whether we hold information about you
- Request access to, or a copy of, the information we hold about you
- Request correction of inaccurate information
- Request deletion of your information, subject to legal or licensing recordkeeping requirements
- Withdraw consent to be contacted at any time
Washington residents: Because referral information may include health- or disability-related information about a prospective resident, certain requests submitted through this Site may involve “consumer health data” under Washington law. You have the right to withdraw consent for the collection or sharing of such information and to request its deletion, subject to our recordkeeping obligations as a licensed care provider.
To exercise any of these rights, contact us using the information in Section 9 below. We will respond within a reasonable time.
Children’s Privacy
Our Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site.
Third-Party Links
Our Site may contain links to third-party websites, including our referral and application forms hosted on ClickUp. This Privacy Policy does not apply to those third-party sites, and we encourage you to review their privacy policies separately.
